North Shore Lactation agrees to accept the payer's (insurance company) negotiated rate. If payer assigns any co-pay, deductible or other responsibility to the patient, then North Shore Lactation will send an invoice directly to the email address provided by the client and client agrees to promptly pay such amount. North Shore Lactation will request and hold a client's credit card on file. North Shore Lactation will never charge your card without advance notice and a written, detailed explanation.

I understand that it is my responsibility to contact my insurance company prior to the consult to verify my benefits and that I will be responsible for all of North Shore Lactation's charges not covered and paid by insurance.

The patient's responsibility will never exceed the self-pay rate of $250 for an initial appointment and $200 for follow-up appointments.

Given the often urgent nature of lactation support, North Shore Lactation kindly asks for 24 hours notice should you need to cancel or reschedule. If cancellation is received within 24 hours of your scheduled appointment, a fee of $50 will be invoiced.
